Final Forms Athletic Registration-2023 Spring Sign Ups Now Open

We are very excited to announce that the Greene Athletic Department will be transitioning from FamilyID to FinalForms, an online registration and data management service. This transition will start in January 2023 and will be used for all Spring 2023 sports registrations.  FinalForms allows you to complete and sign athletic participation forms for your children. The most exciting news is that FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice. FinalForms also pre-populates information wherever possible, for each of your children, saving you time. You may review your data at any time to verify it is current. You will be required to sign your forms once per year and after any update. If you register more than 30 days before the start of the season you will receive an email starting you must complete the health history update.  This must be done within 30 of the start of each sports season.  Note: students will have to sign in as well as they are forms they have to acknowledge.  They will use the student tab and credentials you selected when you registered them as a parent/guardian.  Please use their school email addresses.

We are asking that ALL parents of athletes use FinalForms. You will be required to sign your forms once per year and after any update Please register and follow the prompts to create your account, register your student/child(ren) and sign your forms. The registration can also be found on our athlete page. If you require any support during the process, scroll to the page bottom-right and click “Get Help”.

There are a lot of features in FinalForms that will help improve communication and efficiencies in registering and approving athletes, notifications when athletes are absent from practice and games, email chains to communicate important information, and digital access to emergency contacts and  medical information.  See links below for short videos that highlight some of the above features that coaches and parents will benefit from.  We are excited to bring this product to our athletic department.

Practice Schedules

Sports Physicals:
The prior waiver for sports physicals has ended.  All athletes will need a recent sports physical before being eligible to participate.  

A physical done by your primary care provider does not fulfill this requirement. If a physical was not completed at school, you must make at appointment at UHS Primary Care in Greene before the start of the sport season. 656-4115.

Family ID Athletics Registration

Welcome to Greene Athletics 2022-2023 Sports Registration. FamilyID will open on 9/24 for the following modified sports: 7th & 8th grade boys basketball and 7th & 8th grade girls volleyball. 

For your student to be cleared to play, you must:

  1. Complete FamilyID Form which includes important health information reviewed by the Nurse's office prior to each season as well as mandatory policies and agreements. 

 

  1. Provide a Current Physical: Either upload a current image of physical to the FamilyID sports registration form or bring a copy to the Health office. Physicals expire 12 months after the date of health exam. No athlete will be permitted to play or tryout with an expired physical.

** Health Updates are a NY state mandate and are a different form than the sports physical.  This form is included on your FamilyID online registration.

** No student-athlete will be allowed to participate on the first day of practice unless ALL of the required documents are reviewed and signed; and all health forms are current.

ONLINE CPR AND FIRST AID 
If anyone needs a recertification: go to: topforeducation.us, scroll down home page on left see: coaches view prior to class.  View all videos and take both CPR/First Aid tests.  New coaches will need to complete their training via Zoom and take a skills tests with our "Annie" and training AED.

Coaching Classes: When signing up for class you need athletic director prior approval. Select "contracted" class.  Cost: $75.
