All students planning on participating on any interscholastic sports team for the 2013-2014 school year are required to have a sports physical completed by our school physician.  Our school physician, Dr. Martin Masarech, will be performing sports physicals at school beginning May 8, 2013.  You can print the athletic health history form from the link above.  Once signed by the parent, turn it in to Mrs. Dutcher, MS/HS school nurse.
